To apply you need to submit a completed and signed request form. The request package includes an information booklet and a request form. Please be sure to read the information booklet.

Report any special needs you have on the request form.

Please follow the request process to ensure that your request will be complete, including your and your spouse/partner's (if applicable) signatures.

Special circumstances, such as seniors in a long-term care facility or travel out of the province, may affect the application.
