Birth or Baptismal Certificate

When applying for Alberta Seniors Benefit, attach a copy of your Canadian birth or baptismal certificate. Do the same for your spouse/adult interdependent partner if applicable. If you or your spouse/adult interdependent partner (if applicable) does not have a Canadian birth or baptismal certificate, photocopies of one of the following documents are also accepted:

• Canada Entry Documents
• Canadian Citizenship Document
• Old Age Security Notice of Entitlement letter
• Status Indian Band Card
• Passport
