All sections of the application need to be completed to determine if you are eligible for a benefit under the Alberta Seniors Benefit program.

If you have a spouse, all spousal information must also be completed. The term spouse/partner applies to both a married spouse and a adult interdependent partner. When applying for Alberta Seniors Benefit as part of a senior couple, the older of the two is referred to as the Applicant, and the younger as the Spouse/Partner.

When applying for Alberta Seniors Benefit, attach a copy of your birth certificate or baptismal certificate. Also, attach proof of eligibility for Old Age security.

Your benefit will be deposited directly into your bank account. You will need to attach a blank personalized cheque, pre-printed by your bank/financial institution, with your name, current address and bank account number. Write VOID across the cheque.
