Seniors who have previously applied for the Alberta Seniors Benefit program or the Optical Assistance for Seniors program are automatically enrolled for the Dental Assistance for Seniors program. Seniors who have not applied to either of these programs will need to complete a Dental Assistance for Seniors application form.

To apply for the Dental Assistance for Seniors Program, fill in and mail the application form to the address below. The application form is available (below) in portable document format. If you do not have Adobe Reader to open portable document format files, you can download it for free, or call the Alberta Seniors Information Line at 1-800-642-3853 for an application form to be mailed to you.
