Complaints about Accommodation Standards and Licensing

• Accommodation Standards: For concerns or complaints about accommodation standards not being followed or about unlicensed supportive living settings providing accommodation and support services for four or more adults: call toll-free 1-888-357-9339.
  • You can appeal the decision of the complaints officer by submitting a completed  Complaint Decision Appeal Form.
• Suspected Abuse: For complaints about abuse in a publicly funded facility: call toll-free 1-888-357-9339 (Protection for Persons in Care)

Please have the following information available to help us address your concern or complaint:

1. Name and address of the accommodation;
2. Name of the municipality where the facility is located;
3. Name of the organization operating the facility;
4. Name of the accommodation manager and his or her phone number;
5. A brief summary of your concern or complaint and when it occurred; and
6. Names and titles of people you have spoken to about your concern or complaint (e.g. manager, senior manager, board member) and their involvement in trying to resolve your concern or complaint.
